MyHealthPoint is a comprehensive health and wellness management toolkit including portals for patients, caregivers and clinicians, mobile applications, electronic health record interfaces and advanced data analytics and visualization. The MyHealthPoint platform helps improve user health by better managing wellness goals, automatically collecting remote biosensor information and providing comprehensive care team communications.
Healthx selected MyHealthPoint because it provides a series of proactive population management tools to providers and payors in a cost effective software-as-a-service model used by both companies. MyHealthPoint provides online logging, automated alerts and reminders and a clinical dashboard that triages the entire patient population. Solutions are included for health and wellness as well as disease states such as diabetes, CHF and COPD.
MyHealthPoint is fully integrated with major mobile operating systems including Android, iOS and Windows Phone. This allows the smartphone or tablet to gather, display and update a wide variety of health data points. Demonstrated results include reduced costs, improved outcomes and an increase in the overall quality of care.
