Mobile HealthCare Connections and TLC Health Network Launch Telemonitoring Initiative


BUFFALO, N.Y., Sept. 12, 2012 (GLOBE NEWSWIRE) -- Mobile HealthCare Connections (MHC) announced a major partnership with TLC Health Network (TLC), an affiliate of Lake Erie Regional Health System, to deploy its state-of-the-art home telehealth technology among TLC's patients. MHC is at the forefront of integrating technology with personalized attention and leading an industry working to curb re-hospitalization rates, improve patient outcomes, and provide customized care directly to the patient's home.

The contract between MHC and TLC will initially involve one hundred kits with state-of-the-art telehealth devices monitored by MHC's personnel for six months to assess the positive impact on patient outcomes and reducing the costs of unnecessary and unplanned doctor and emergency room visits as well as hospital readmissions. Mobile HealthCare Connection's unique business model combines telemonitoring care, pharmacy services and nurse practitioner services all under one roof, delivered to the patient's home. As part of the contract with TLC, they will monitor the patient vital signs including blood pressure, blood oxygen, pulse, body weight, and glucose levels on an as-needed basis.
