BUFFALO, N.Y., Sept. 12, 2012 (GLOBE NEWSWIRE) -- Mobile HealthCare Connections (MHC) announced a major partnership with TLC Health Network (TLC), an affiliate of Lake Erie Regional Health System, to deploy its state-of-the-art home telehealth technology among TLC's patients. MHC is at the forefront of integrating technology with personalized attention and leading an industry working to curb re-hospitalization rates, improve patient outcomes, and provide customized care directly to the patient's home.

The contract between MHC and TLC will initially involve one hundred kits with state-of-the-art telehealth devices monitored by MHC's personnel for six months to assess the positive impact on patient outcomes and reducing the costs of unnecessary and unplanned doctor and emergency room visits as well as hospital readmissions. Keith Blakely, CEO of The InVentures Group and a founding partner of MHC, commented, "When you begin to focus on preventive care, customized to each individual patient, you end up with a much more efficient healthcare system. I am excited to see MHC's technology in action, improving the daily lives of patients at the TLC Health Network Lake Shore Hospital."

Mobile HealthCare Connection's unique business model combines telemonitoring care, pharmacy services and nurse practitioner services all under one roof, delivered to the patient's home. As part of the contract with TLC, they will monitor the patient vital signs including blood pressure, blood oxygen, pulse, body weight, and glucose levels on an as-needed basis. MHC's President, Cory Shaffer discussed the arrangement: "It's great to see one piece of our offering, our telemonitoring kits, being deployed at an ever-increasing number of healthcare facilities around the country.

We already know the highly positive benefits to the patients.  This arrangement will allow us to continue to measure and quantify the economic benefits of our integrated approach to health care costs."

Linda Summers, Chief Operating Officer of TLC Health Network, had this to say, "Our organization is always looking for opportunities to provide the best possible experience and outcomes for our patients. By working with MHC we can achieve superior results and patient satisfaction at reduced costs - the partnership is a win-win for everyone involved."
