Leaders from Mosaic Life Care will be in Cerner's booth Feb. 24 sharing their first-hand experience as an accountable care organization, and the results they've achieved with Cerner's population health solutions. On Feb. 26, physicians and clinicians from Advocate Health Care will be in Cerner's booth highlighting their strategy for population health, as well as the outcomes they've generated. Both clients will be available to speak to the ways their organizations have leveraged Cerner® solutions to advance their population health initiatives.

Joe Boyce, MD, Mosaic Life Care chief information officer and chief medical information officer, will share the provider's experience as an accountable care organization, and discuss participating in the Lewis and Clark Information Exchange. LACIE is a non-profit entity that's been exchanging data since 2010 between independent healthcare organizations and providers. Participants are from Kansas and Missouri, and range in size from small independent physician practices to large academic medical centers.
Mosaic Life Care representatives will also be available to speak about their use of the Cerner member portal and enterprise data warehouse.
Mosaic Life Care has an inpatient facility and 52 outpatient facilities serving 23 counties in northwest Missouri, northeast Kansas and southeast Nebraska. Cerner SmartRegistries is a cloud-based solution that provides the capability to stratify patient populations based on risk, conditions and attributed physicians. In addition, the registries enable care managers to quickly determine what key quality measures specific to the designated condition of the patient have been met.
