Cerner Clients Share Population Health Strategies, Results at HIMSS14

KANSAS CITY, Mo., Feb. 21, 2014 (GLOBE NEWSWIRE) -- Visitors will experience the "now" and the "next" in health care in the 13,300 square foot Cerner booth (6965) at the Healthcare Information and Management Systems Society annual conference in Orlando, Fla.

Leaders from Mosaic Life Care will be in Cerner's booth Feb. 24 sharing their first-hand experience as an accountable care organization, and the results they've achieved with Cerner's population health solutions. On Feb. 26, physicians and clinicians from Advocate Health Care will be in Cerner's booth highlighting their strategy for population health, as well as the outcomes they've generated. Both clients will be available to speak to the ways their organizations have leveraged Cerner® solutions to advance their population health initiatives.

"Population health is more than improving care, lowering costs and increasing patient satisfaction. It's also developing intelligence to predict what outcomes may be likely and prevent them from occurring through timely care interventions. These clients are extraordinary examples of what that looks like," said Matthew Swindells, Cerner senior vice president. "We're helping organizations manage the health of their populations' one person at a time. Cerner's solutions enable organizations to know their population, to engage members and to improve care."

Mosaic Life Care, St. Joseph, Mo.

Joe Boyce, MD, Mosaic Life Care chief information officer and chief medical information officer, will share the provider's experience as an accountable care organization, and discuss participating in the Lewis and Clark Information Exchange. LACIE is a non-profit entity that's been exchanging data since 2010 between independent healthcare organizations and providers. Participants are from Kansas and Missouri, and range in size from small independent physician practices to large academic medical centers.

Mosaic Life Care representatives will also be available to speak about their use of the Cerner member portal and enterprise data warehouse.

Mosaic Life Care has an inpatient facility and 52 outpatient facilities serving 23 counties in northwest Missouri, northeast Kansas and southeast Nebraska. Mosaic Life Care joined the Mayo Clinic Care Network in 2012, extending the Rochester, Minn.-based provider's expertise, resources and innovation to the physicians and providers at Mosaic Life Care.

"Mosaic Life Care's use of Cerner's member portal is a great example of the importance of member engagement as a component of an effective population health management strategy," said Swindells. "The portal plays a vital role in this strategy, providing a secure avenue for patients to directly interact with their care team."

Advocate Health Care, Downers Grove, Ill.

Tina Esposito, vice president, Advocate Health Care Center for Health Information Services, will share the organization's population health story, health care innovations and recent implementation of Cerner SmartRegistries™. Cerner SmartRegistries is a cloud-based solution that provides the capability to stratify patient populations based on risk, conditions and attributed physicians. In addition, the registries enable care managers to quickly determine what key quality measures specific to the designated condition of the patient have been met.

"Cerner SmartRegistries was developed through our partnership with Advocate," said Swindells. "They are working to move beyond automating health systems to managing a person's health."

Advocate Health Care operates more than 250 sites of care, with more than 6,000 affiliated physicians. Advocate's partnership with Cerner has become a model for health care organizations looking for a team-based approach to care, effectively managing patients across multiple care sites.
