Death of an inmate at Saskatchewan Penitentiary


PRINCE ALBERT, SASKATCHEWAN--(Marketwired - June 7, 2017) - Correctional Service Canada

On June 7, 2017, Daniel James Tokarchuk, an inmate from Saskatchewan Penitentiary died while in our custody.
Emergency services were called. The offender was taken to the hospital where he was pronounced dead at 4:24 a.m.

At the time of his death, Mr. Tokarchuk, 44 years old, had been serving an indeterminate sentence since December 16, 2004 for Second Degree Murder, Mischief in Relation to Other Property and Failure to Comply with Appearance Notice.

The inmate's next of kin have been notified of his death.

As in all cases involving the death of an inmate, the police and the coroner have been notified, and Correctional Service Canada will review the circumstances of the incident.
