Matrix Medical Network Shares How In-Home Visits Impact Care at AHIP Medicare Conference

Two Panels of Distinguished Healthcare Leaders Discussed How They Impact and Improve Member Care Through In-Home Visits


SCOTTSDALE, Ariz., Oct. 6, 2014 (GLOBE NEWSWIRE) -- Matrix Medical Network® (Matrix), the leading provider of in-home health assessments, presented two special information sessions on improving member care with non-traditional care delivery models at America's Health Insurance Plans (AHIP) Medicare Conference on Sept. 28 – 30, in Washington, D.C.

Matrix conducted a pre-conference workshop on Proven Steps to Accelerate Star and HEDIS Measures that featured panelists Doug Fulton, vice president of Highmark's Medicare Stars & Quality Rating System organization, Mary Vogt, president of Home Access Health, and Deb DiCicco, MS, FNP-C, director, clinical design for Matrix. During the workshop, panelists discussed their experiences with in-home screening and the insights gained from these tests including:

  • 23% of tested diabetics indicate possible kidney disease
  • 10% of all individuals tested should have follow up to rule out possible colon cancer
  • 28% of tested diabetics had an elevated A1C with 4% having a level of 9.9 or above

Early detection of these potentially high-risk health conditions enables health plans to help members manage their care before a more serious health event occurs.

MatrixVice President of Chronic Care Heidi Wold, MSN, ARNP, ANP-BC, led the session The New Care Delivery: Reinventing Care Coordination and Management. Ms. Wold discussed how innovative care initiatives are creating new opportunities for health plans to learn more about the care needs of its individual members and its general member populations. New care delivery models focus on providing in-home care, by an entire care team, to help members with complex medical conditions. The information collected from providing this type of individualized care is the ingredient that can lead health plans to have a greater impact on member care.
