ASCP Announces Forum on Post-Acute Transitions of Care

ALEXANDRIA, VA., Feb. 26, 2015 (GLOBE NEWSWIRE) -- The American Society for Consultant Pharmacists (ASCP) has announced it will host the "ASCP Forum: Pharmaceutical Care and Post-Acute Transitions," a discussion for all stakeholders in long-term care on this important issue. This inaugural Forum will provide insights from the nation's top thought leaders in the post-acute care pharmacy industry.
