Caravan Health Announces Second Annual Accountable Care Symposium

Join hundreds of accountable care pioneers to learn how rural hospitals, clinics, small practices and independent providers can achieve better outcomes and higher earnings

Kansas City,

Austin, Texas, July 12, 2017 (GLOBE NEWSWIRE) -- Caravan Health is excited to announce its second annual Accountable Care Symposium, the premier learning and networking event for current and future participants in Advanced Payment Models. The landmark event returns to Phoenix, Arizona on December 6 and 7 at the Pointe Hilton Squaw Peak Resort, where hundreds of healthcare professionals will come together to facilitate high-level learning and thoughtful discussion about the advancement of value-based payment systems.

The symposium will feature industry thought leaders as they shed light on the key-issues effecting the value-based care landscape. Attendees will participate in breakout sessions during the two-day event, where field experts will share their experiences and strategies for effective population health management.

“We’re thrilled to build on the tremendous success of our first symposium,” said Lynn Barr, CEO of Caravan Health. “This year will feature more breakout sessions which will allow ample time for hands-on learning and exchange of ideas. The focus of the symposium is to ensure all participants are equipped with the resources they need to succeed in 2018.”

The first day of the symposium will be dedicated to policy and physician engagement before breaking into specialized tracks that will address the real-world concerns of new and experienced Accountable Care Organizations (ACOs), and Transforming Clinical Practice Initiative (TCPI) and Comprehensive Primary Care Plus (CPC+) participants. The second day will highlight how practices can drive their Advancing Care Information (ACI) scores and cover Behavioral Health Integration (BHI), including practical content about how to address the challenges of opioid addiction. Taken together, the content will enable practices to implement workflows that will achieve the highest scores regardless of which program they joined or how far along they are in the transformation process.

The meeting will convene independent practice associations, small practices, rural health systems, community hospitals, FQHCs and multi-specialty groups; together with hundreds of current ACO participants including primary care physicians, community hospital CEOs, nurse care coordinators and executive leaders. Many newly accepted ACO participants will attend, joined by physicians and hospital leaders who are preparing to enter into Advanced Payment Models in 2018.

Those who attended last year’s symposium said:

“Great time to network and connect with peers, and learn about what is going on in the next year.”

“I was impressed by the turn-out, and that everyone participated in every session. There was a broad representation from across the country as well as from various roles - practice managers, physicians and CEOs. It was great to hear the interaction.”

“The information and quality of speakers was fantastic.”

Registration for the symposium opens Tuesday, August 1, 2017. Attendees may take advantage of early bird pricing if they register by September 1, 2017. Early bird rates are $199 for current participants of Caravan Health and the National Rural Accountable Care Consortium; $399 for non-participants. Registration rates after September 1 are $299 for participants of Caravan Health and the National Rural Accountable Care Consortium; $599 for non-participants. The program includes an awards ceremony and dinner, two breakfasts and lunches, speakers, break-out sessions, networking meetings, and access to the resort’s world class amenities.
