SAN FRANCISCO, CA--(Marketwired - Apr 17, 2013) -  Wellframe -- a Boston-based health technology startup dedicated to improving care management -- today announced the unveiling of Wellframe Heart Coach, the first clinical program on the Wellframe Care Management Platform, at DEMO Mobile 2013. The Wellframe Care Management Platform integrates mobile technologies with artificial intelligence and a supportive service to extend the provision of clinical care from the hospital to the home, empowering patients to better manage their health and helping providers thrive in an evolving payment landscape. The Wellframe Heart Coach seeks to improve post-acute care for patients with coronary heart disease and congestive heart failure, leading causes of mortality and drivers of healthcare costs. In addition to the Wellframe Heart Coach, the company is currently working to expand its clinical program offerings into other clinical verticals such as diabetes and mental illness.

The Wellframe Care Management Platform enables healthcare professionals to reach more patients more effectively and more efficiently. Wellframe's Care Management Clinical Programs are designed to amplify the impact of hospitals' existing human resources in order to improve patient health outcomes and manage healthcare costs. Wellframe's mission is to advance a novel approach that helps clinicians more effectively manage the moments in between patient contact with the healthcare system, to help patients feel better connected with their care team and to receive clinical support on a daily basis.

"We're thrilled to have the opportunity to launch the Wellframe Heart Coach at DEMO Mobile 2013. We believe that we're helping to advance a novel approach to care management that will be invaluable for patients and for health care providers," said Jake Sattelmair, Wellframe Co-Founder and CEO. "Unlike many mobile health products, we're really focused on amplifying and extending clinical care, while making valuable clinical resources more accessible to more patients. We're already conducting trials with some of the nation's top academic medical centers and health systems, and the results so far have been extremely positive, with high levels of sustained engagement and perceived value from clinicians and patients alike."

"Having previously run a primary care practice with 10,000 patients under care and closely studied best practice models of care quality improvement, it's very clear that having a tool like this will dramatically improve the efficiency and effectiveness of care management," said Trishan Panch, MD, Co-Founder and Chief Medical Officer. "It is clear that most patients want to do all they can to be healthy and health care providers want to do all they can to support patients in doing the right thing. However, the system and the lack of an effective and efficient means to engage patients other than phone calls and visits make it really hard. We want to change that and we are very excited about the potential of the Wellframe Care Management Platform."

Wellframe recently completed a successful pilot study of the Wellframe Heart Coach with Brigham and Women's Hospital, an affiliate of Harvard Medical School, and South Shore Hospital, and will be launching a research study in conjunction with a major academic medical center in New York City over the next couple months. Additionally, Wellframe plans to roll out the platform to ten early adopter hospitals over the next six months. For more information on Wellframe, please visit
