Conference attendees include senior healthcare leaders in payer, hospital and physician practice organizations. The session topic bolsters the forum's overarching focus on analyzing and transforming data into actionable information to influence quality and cost and address the lack of proven population health management best practices. Schor and Dr. Kudler will provide a practical approach to treating population health management like a team sport by using box scores to work as a system, anticipate and prevent risk, and proactively adjust care management strategies.
