ConcertoHealth Taps Three Executives for Michigan Expansion Team: Dave Tigue, President; Dr. Robert Levine and Dr. Bobby Lee, Regional Medical Directors

Leadership Hires Timed With Introduction of New Care Model for Complex Medical Needs, Medicare-Medicaid Beneficiaries; Four Care Centers Serving Detroit

DETROIT, MI--(Marketwired - Dec 8, 2015) - ConcertoHealth today announced the appointments of Dave Tigue as president of its Michigan operations, Dr. Robert Levine and Dr. Bobby Lee as Michigan regional medical directors, clinical operations.

The appointments were made as the company recently opened three additional Care Centers in the greater Detroit area. ConcertoHealth now operates four Care Center clinics in metropolitan Detroit.

"Tigue is an authority on innovative clinical provider services to Medicare, Medicaid and Dual Health Plans with expertise in improving clinical quality and outcomes for the complex needs patient population," said Michael Carson, ConcertoHealth Chief Operating Officer. "Dr. Levine brings decades of medical director, clinical and private practice experience. Dr. Lee has extensive experience and thought leadership in hospital medicine, geriatrics and palliative care. The team's combined expertise is a significant asset to our growth in Michigan, especially during this growth phase."

ConcertoHealth has more than a decade of experience caring for the Medicare, Medicaid, frail and complex medical needs patients. By merging social work with care management, clinical services, and complex disease management, ConcertoHealth ensures patients and their families receive the individualized support they need to improve health outcomes. The company provides Care Centers for patients who can get to a clinic, in-home care for those who can't, and its hospital- and field-based care teams provide for those in-hospital, in a nursing home, or any post-acute setting.

The company gained a deep understanding of the complexities involved with healthcare delivery and financial risk management of this patient population as both a Medicare Advantage health plan and as a care provider. It was the success of ConcertoHealth's model that led to the company's selection by the State of Michigan Duals Demonstration Medicare-Medicaid Plan program.

ConcertoHealth integrates clinical care with social services to ensure patients and their families receive the individualized support they need to improve health outcomes.

"The ConcertoHealth model provides improved access to healthcare for patients who can get to a clinic, in-home care for those who can't, and field-based care teams for those in any other health care setting, from hospitals to nursing homes," said Tigue. "It's an exciting time to join the ConcertoHealth team. We're changing the face of healthcare, particularly for an often neglected population."

"ConcertoHealth delivers on improving access to primary care services with medical centers in the communities it serves," said Dr. Levine. "This is the solution for improving patient outcomes -- addressing both the social and economic realities that are faced in particular by dual-eligible patients."

"Our approach is that a patient is more than a compilation of symptoms or illnesses, but rather the result of history, culture, lifestyle, living conditions and more," said Dr. Lee. "ConcertoHealth takes into account all aspects of a patient's life and it's an honor to be a part of this innovative, overarching care model."
