The Heart Failure Health Storylines App Supports Self-Care Among People Living With Heart Failure and Congestive Heart Failure (CHF)


BETHESDA, MD--(Marketwired - February 02, 2016) - The Heart Failure Society of America, Inc. (HFSA) kicked off 2016 with the launch of a new app, Heart Failure Health Storylines, to support people living with heart failure, and congestive heart failure (CHF). The app, developed in partnership with patient intelligence and health solutions provider Self Care Catalysts, will provide the Heart Failure Society of America with a mobile and web application powered by the Health Storylines™ platform, which consists of a suite of health tools specially designed to support self-care among people living with heart failure.

The Heart Failure Health Storylines app makes it easy for patients to track their symptoms, sodium intake, vital sign, moods, as well as sync with other apps such as FitBit and Jawbone. It also allows them to receive reminders on when to take their medications, which may help improve treatment adherence. Through tracking their health daily, the app creates a summary of their health called "My Storylines" which enables them to be more informed about their health, and to have a more productive conversation with their care team. Heart Failure Health Storylines is free for all users and is available on the web, the App Store™ and on Google Play™ in the USA.
