WAYNE, PA and LAS VEGAS, NV--(Marketwired - February 26, 2016) - Medecision, the leader in population health management solutions for risk-bearing entities like provider- and payer-sponsored ACOs, hospitals, IDNs and health plans, announced today that healthcare IT trailblazer and Medecision customer, Dr. Terri Steinberg from Christiana Care Health System (CCHS), one of the largest health systems in the mid-Atlantic region, was selected to host a key population health-focused session at the 2016 HIMSS Conference & Exhibition, held in Las Vegas February 29-March 4. With population health management strategy powered by Medecision's Aerial™ platform, Dr. Steinberg will present on the results and evolution of Christiana Care's Bridging the Divides population health initiative, including improved patient outcomes and an annual cost savings of $1.75 million.

Christiana Care's Bridging the Divides program is a CMS-funded platform initiative aimed at improving outcomes and lowering cost in the organization's ischemic heart disease population. Based on lessons learned from the first three years of the program, the health system has expanded the initiative and debuted an advanced technology-enabled care coordination platform within the last year. Dr. Steinberg will discuss how the program, powered by rich patient-level data sets; regressive and predictive analytics; automated care management functions; and risk scoring capabilities from Medecision's Aerial platform has enabled more precise care of the organization's high-risk populations.

"Evolving the Bridging the Divides program has enabled CCHS to grow its population health management initiative by more accurately viewing patients within populations needing certain levels of care coordination," said Dr. Terri Steinberg, chief health information officer and vice president of population health analytics at Christina Care. "This knowledge has led to greatly improved ratios of care managers to patients, promoted more efficient utilization and, importantly, proven to us that improved care coordination processes result in dramatically improved outcomes."

Results of the program showed CCHS areas of strengths and weakness in their pilot initiative. These key results now inform a second phase of the organization's population health efforts, including:

  • Enhanced patient satisfaction, including 91 percent of patients receiving the right amount of follow-up after discharge and 88 percent were more optimistic about their future;
  • A streamlined care management process; and
  • The need to expand the program into additional high-risk populations to measure larger numbers of patients.

"The successful transition into a value-driven environment requires smart partnering decisions and a strong understanding of the disruptive forces impacting care delivery and how to position the organization most effectively moving forward," said Deborah M. Gage, president and chief executive officer at Medecision. "We are thrilled to be working with innovative health systems like Christiana Care as their trusted partner-convening the insight and expertise they need to properly assess risk, build effective care management processes and realize population health goals."

Dr. Steinberg's presentation is titled "The Evolution of Population Health Management." She'll be presenting on Tuesday, March 1 from 10:00-11:00 a.m. PT in the Sands Expo Convention Center, Palazzo I.
