SAN FRANCISCO, CA and BOSTON, MA--(Marketwired - May 09, 2016) - Grand Rounds, the leader in identifying high quality health care providers and connecting patients with these physicians, and Iora Health, an innovative health system delivering high impact, relationship-based care, today announced a relationship in which Iora Health will use Grand Rounds' tools to identify the best physician specialists for Iora Health's primary care patients.

This relationship advances patient care by combining Iora Health's unique care delivery model with Grand Rounds' data-driven approach to identifying physician specialists of exceptional clinical judgment and skill.

"Iora's vision is to transform health care starting with primary care, and a key tenet to achieving our vision is ensuring our patients get the care they need in -- and outside -- of our walls," said Marc-David Munk, M.D., vice president of accountable care at Iora Health. "We are pleased to partner with Grand Rounds to help guide our patients to high quality specialists, as rigorously selected via the Grand Rounds platform -- helping to ensure each patient receives the very best and most appropriate care possible."

Iora Health provides a new model of health care for a diverse array of patients, including seniors on Medicare Advantage and those enrolled on public exchanges. The combination of Iora Health's care excellence and Grand Rounds' specialist expertise will support the delivery of effective patient care, as well as improved financial performance of the population.

"We're excited to partner with an innovative care group like Iora Health that is delivering better health care through a sophisticated and comprehensive care model," said Jacob Best, head of medical networks at Grand Rounds. "Iora Health shares Grand Rounds' mission of getting all patients on the path to better health, and we see this relationship as yet another way to offer patients evidenced-based, empathy-centered care."

Grand Rounds pairs patients with the physician specialists most uniquely qualified to help them, nationally and/or locally. The Grand Rounds quality algorithm has rated more than 96 percent of all active U.S. physicians. This unique data set will be used by Iora Health to guide patients to top-quality local care, ensuring their interactions with specialists are consistent with the highest standards of care.

This service will be available in six of Iora Health's covered markets, including Denver, Phoenix and Seattle.
