Excellus BCBS Chooses Wellframe Mobile Platform to Extend Integrated Care Management and Quality Improvement Services

BOSTON, MA--(Marketwired - August 03, 2016) - Wellframe today announced that Excellus BlueCross BlueShield has chosen to use its mobile-enabled member engagement platform to enhance the reach and effectiveness of its Health Care Improvement team (HCI), which integrates care management and quality improvement programs to improve health outcomes, improve patient experience, and close gaps in care.

Establishing enduring relationships with members who live with chronic disease is critical to achieving member engagement and empowerment. These relationships provide a vehicle to deliver effective care management interventions, which lead to improved clinical outcomes and satisfaction. The Wellframe platform will enable Excellus to develop and deliver innovative care programs for their chronic disease populations through a mobile app, which will extend the reach and capacity of the organization's existing care management resources.
