Primaria Health Selected to Participate in CMS Next Generation Accountable Care Organizations (ACOs) Model

Community Health Network’s providers deliver better, more affordable care to Central Indiana’s growing Medicare population

Indianapolis, IN, Feb. 19, 2018 (GLOBE NEWSWIRE) -- Primaria Health, a primary care management services organization, has been selected to participate in the Medicare Next Generation Accountable Care Organization (ACO) Model. Primaria provider partners will now have additional resources from the Centers for Medicare and Medicaid Services (CMS) to continue providing higher-quality, more affordable care for its approximately 30,000 Medicare.  Primaria will continue to provide its physician partners with the solutions and services they need to improve outcomes, enhance patient and physician satisfaction, and lower the total cost of care.

“We are committed to finding ways to improve the health and wellness of our aging population, and this new Medicare program is a great step forward in doing just that” said Doug Stratton, chief executive officer, Primaria.

Primaria initially formed an ACO in 2017 and is the largest network of primary care physicians in Central Indiana with 400 provider partners. Moving to a Medicare Next Generation ACO in 2018, the network can expand its care management tools and resources to better address the needs specific to seniors as well as support them in achieving performance metrics for value-based contracts.

“In a little over two years, the providers at Community Health Network have demonstrated what is truly possible in terms of improving clinical outcomes while reducing the cost of healthcare.  I am thrilled that we are now applying our model to a broader section of the Medicare population,” said Tim Barry, chief executive officer, VilllageMD, a national provider of primary care and a Primaria partner.

Primaria’s provider partners will be building upon their demonstrated clinical improvements in the areas of diabetes, congestive heart failure (CHF), and chronic obstructive pulmonary disease (COPD) and other complicated health conditions. Using Primaria’s high-value clinical care model, admission and readmission rates were 20 to 45 percent lower than the community, Medicare costs were 20 to 45 percent lower than the community, and commercial costs were 15 to 20 percent lower than the community.

Primaria brings sophisticated data analysis to identify patients at most risk for a variety of health conditions and complications, as well as those living with chronic conditions.

“We are honored to be selected to participate in this program,” said Patrick McGill, M.D., Senior Vice President, Clinical Strategy, Community Health Network. “Improving lives in our Medicare population is so critical because this growing population has a greater risk for health issues.”

Primaria is a joint venture between VillageMD, a national provider of primary care management services, and Community Health Network, a healthcare system based in Indianapolis.
